Methods of orthodontic treatment

Different types of appliances which are both fixed and removable are used to help in the movement of teeth, to retrain muscles which affect the growth of the jaws. The appliances which are used in the orthodontic treatment work by placing gentle pressure on the jaws and teeth. Based upon the severity of your problem and the condition of your teeth the orthodontic approach which best suits the patient can be determined.

The fixed appliances include:

  • Braces: These are the most common fixed appliances which usually consist of bands, wires and brackets. For a comprehensive orthodontic treatment, metal wires or ‘Jushi ‘are inserted into the orthodontic brackets. These metal wires are made from stainless steel or from a more aesthetic ceramic material. The wires inserted interact with the orthodontic brackets to move the teeth into the desired positions.
  • Special fixed appliances: These are used to control thumb sucking or tongue thrusting. These appliances are attached to the teeth with the help of bands. They are usually used as a last resort as it causes difficulty in eating.
  • Fixed space maintainers: In the case of premature loss of baby tooth, a space maintainer is used to keep the space open till the eruption of the permanent tooth. Here a band is attached to the tooth next to the empty space and a wire is extended to the tooth on the other side of the space.

The removable appliances include:

  • Invisalign and other removable appliances: This is another method of orthodontic treatment wherein plastic aligners are inserted to help in the movement of teeth. Additional appliances that are removable like plates, headgear, expansion appliances and many other devices may also be used to help in the movement of teeth and jaw bones.
  • Dentofacial Orthopedics: This orthodontic treatment includes functional appliances and is mainly used for children in the age group of 5 years to 14 years. This treatment mainly aims at modifying the dimensions of the jaw. This is usually followed by fixed multi-bracket therapy which mainly consists of full braces that are used to align the teeth and also to refine the occlusion.
  • Removable space maintainers: Some orthodontists also work on reconstructing the entire face rather than just focusing exclusively on the teeth alone. After an active orthodontic treatment course, the patients will be made to wear retainers. The retainers help to maintain the teeth in their improved and aligned positions while surrounding the bone reforms that are present around them. The retainers are initially worn full time for a period of a few days to a year. Later the patients are advised to wear the retainers partially which is typically during night time. They will continue this as long as the orthodontist recommends them to do so. It is best advised by many orthodontists for the patients to continue wearing the night time or part-time retainers for many years after the orthodontic treatment is done. This ensures that the teeth alignment is retained. In the case of Adult orthodontic patients they will most likely need lifetime retention to maintain their teeth intact.
  • Jaw repositioning appliances- They are also called splints which are worn on the top or lower jaw and help to train the jaw to close in a more favorable position.
  • Lip and cheek bumpers: These are designed to keep the cheeks or lips away from the teeth to relieve the pressure.
